Terrifier 3 continues the horrifying story of Art the Clown, who returns to terrorize his victims after being decapitated by Sienna Shaw. Despite being without a head, Art steals the head of a police officer and carries on with his sinister plans.
Meanwhile, Victoria Heyes, a survivor now possessed by the Little Pale Girl, has just given birth. Together with Art, they murder a nurse and a guard at the psychiatric hospital before escaping.
Five years later, Sienna is released from a mental health facility and goes to live with her aunt Jess and her family. Although she tries to move on with her life, Sienna is still haunted by guilt, feeling responsible for the death of her best friend, Brooke.
Despite everything, Sienna makes an effort to reconnect with her younger brother, Jonathan. The siblings strive to move forward and leave their dark past behind.
Elsewhere, Art and Victoria awaken from their long slumber and resume their violent rampage. Art slaughters Jonathan's friends at college and disguises himself as Santa Claus to launch a bloody attack at a mall just before Christmas.
Sienna senses the looming threat, but her family dismisses her fears. Determined to stop Art, Sienna returns to the site of past horrors and retrieves her father's sword.
